East Bay Community Action Program (EBCAP) has an immediate opening for a full-time Community Health Worker for State Health Assistance Program (SHIP) program at our location at 100 Bullocks Point, East Providence, RI,
The Community Health Worker (CHW) will coordinate all day-to-day activities to execute the Integrated Senior Health Insurance Program and will address the unique needs of Medicare beneficiaries, with a specific focus on the mitigation of and recovery from COVID-19, as well as education on the choices and options available for comprehensive Medicare coverage. Provide culturally accessible information to promote COVID-19 testing, vaccinations, and boosters; assist with connecting Medicare beneficiaries to resources that will promote successful community living and prevent hospitalizations, and provide health insurance and other benefits navigation. The CHW will provide services in community-based settings to enhance accessibility to resources, and to provide outreach and education of EBCAP and SHIP services at community gathering places frequented by Medicare beneficiaries (e.g. senior centers, housing sites, and homebound seniors), events, testing sites, religious organizations, workplaces, sports activities, and other local spaces and high-traffic areas where people congregate.
In addition to COVID 19 mitigation strategies, educate and counsel Medicare beneficiaries regarding Medicare choices, options etc. Assist community members with Food Pantry assessments, completing SNAP, LIHEAP, WIC, health education and wellness programs, and other social program applications. Cultivate and maintain relationships with organizations that assist elders and/or disabled individuals.
This position requires a High School diploma or equivalent. Previous equivalent work experience in a similar field is preferred. Annual SHIP Counselor certification is required. Current certification through the RI Certification Board (RICB) as a Community Health Worker (CHW) or certification within six months of hire is required.
