East Bay Community Action Program (EBCAP) is seeking a full-time Family Advocate Generalist. The Family Advocate will work closely with the School-Based Family Navigator to connect identified students/families with East Bay Community Action Program (EBCAP) services, with special emphasis on behavioral healthcare needs. Other potential supports/services identified during assessment/review may include, but are not limited to: primary care, wrap-around/social services, youth center engagements, etc., with the goal to enhance and improve student and family stability and success. The Family Advocate will provide clinical and/or administrative support to behavioral health staff (case managers, nurses, prescribers, etc.) as appropriate.
EBCAP is offering a two-step sign-on bonus totaling $1,000 to external candidates.
Facilitate the scheduling of appropriate behavioral health services through EBCAP’s Access Department. Assist behavioral health staff with providing appointment reminders, education to the families, etc., to encourage participation in treatment/services.
Foster family development, through identification of strengths and needs, goals, and resources by conducting Family Assessment and establishing a Family Development Goad Plan. Monitor progress of plan with family, clinical staff, etc., as needed.
Provide case management services that link clients to services that will lead to the stabilization of their situation. This includes interviews and assessments of homeless families for placement into EBCH homeless program.
Maintain ongoing contact with families by providing scheduled office, community, and in-home visits.
MINIMUM REQUIRED EDUCATION
Bachelors Degree in Human Services’ field OR equivalent/relevant experience required. Must have experience with behavioral health assessments.
