East Bay Community Action Program (EBCAP) is seeking a full-time School-Based Family Navigator.  The School-Based Family Navigator will provide appropriate review and assessment of identified school-based students and families, in order to facilitate coordination with Family Advocate Generalist, who ensures direct connection to East Bay Community Action Program (EBCAP) services, with special emphasis on behavioral healthcare needs. Other potential supports/services identified during assessment/review may include but are not limited to primary care, wrap-around/social services, youth center engagements, etc., with the goal to enhance and improve student and family stability and success.  Liaison with Family Advocate Generalist to ensure connections to appropriate services are scheduled and carried out.

EBCAP is offering a two-step sign-on bonus totaling $1,000 to external applicants. 

Assist in the identification of at-risk families by actively participating in school partnership meetings and activities.  Proactively work with school special services (guidance, adjustment counselor, etc.) staff and creatively outreach family members in order to assess and link to needed services.

Continually liaison with behavioral health staff regarding eligibility criteria, access availability, potential interventions, etc. to ensure appropriate referral/connection is made.

Creatively outreach to families in the community in order to fully engage, assess and intervene in the family’s needs. This will require flexible hours for community outreach/intervention.

Provide individualized case management to students/families by responsively referring and following up with needed resources and services.

Coordinate provision of services through EBCAP’s Family Advocate Generalist, to ensure appropriate services are scheduled and carried out with a special focus on behavioral health services.

Monitor students/families for situational improvement.  Provide additional assessments as appropriate to supplement initial recommendations if necessary.


Bachelor’s Degree in Human Services field OR equivalent/relevant experience required.  Must have experience with behavioral health assessments.
