Summary: Provide person-centered counseling services to aging adults 55 and older, adult with disabilities and their caregivers. Provide conflict-free information and one-on-one counseling to help them make informed decisions about options available to them.

Duties and Responsibilities:

 

Follow scope of work provided by OHA and use it as guide for accomplishments
Attend all required trainings by EOHHS and OHA and assist individuals with materials, as needed
Handle Incoming/Outgoing Contacts/Follow-up
Attend meetings as necessary, regarding the project
Attend Outreach events representing The Point
Work with team members at the POINT, the POINT Regional Partners and OHA to refine strategies to address required populations across Rhode Island
Continue with a plan to coordinate and facilitate communication and coordination between SHIP/POINT agencies, MME and BEC
Track all data in ServicePoint and/or Stars
Keep a paper file with supportive documentation for all options counseling cases
Work closely with other Options Counselors, under this project, to coordinate activities and responsibilities to ensure statewide access to person-centered counseling for all affected beneficiaries.
Gather input from stakeholders about the effectiveness of the public awareness campaign regarding person-centered counseling regarding Medicare/Medicaid benefits to eligible beneficiaries and make improvements to the campaign, when appropriate
Continue attending  meetings and trainings as required
Assist with training SHIP/Regional POINT counselors
Provide conflict-free person-centered counseling   to eligible individuals
Evaluate and refine trainings, as needed
Assist in training SHIP/ADRC counselors and BHDDH Support Coordinators, as needed
Coordinate and facilitate communication and coordination between SHIP/ADRC agencies and MME enrollment help line
Work with referral network between BHDDH Support Coordinators and MME enrollment line
Collaborate with community mental health organizations to reach SPMI population
Keep United Way staff informed of daily activities
Provide monthly written summary of activities
Complete time sheet weekly
Tier I Options Counselor

Includes all Duties and Responsibilities listed above
Certifications Required:

SHIP certification within six months of hire
Tier II Options Counselor

Includes all Duties and Responsibilities listed above. (Essential and Tier I)
Additional Responsibilities Include:

Assist with training for any new options counselors and SHIP Volunteers
Coordinate volunteer hours and duties
Certifications Required:

SHIP Certification within six months of hire
CAC Certification within one year of hire
Tier III Options Counselor

Includes all Duties and Responsibilities listed above. (Essential, Tier I and II)
Additional Responsibilities Include:

On board training of new United Way employees teaching them about The Point and 211
Training new Point employees on duties and responsibilities
Be responsible for coordinating outreach and presenting on behalf of The Point
Certifications Required:

SHIP Certification within six months of hire
AIRS Certification within two years of hire
Navigator Certification within two years of hire
Knowledge, Skills, and Abilities:

Professional training and/or experience in Social Work, Psychology, Counseling or related field
Experience and knowledge of Medicare & Medicaid and/or other insurance plans
Experience in working with groups and speaking publicly
Experience in one-on-one counseling and/or client-focused activities
Must have own transportation and be willing to travel throughout the state, as needed
Ability to work independently and to meet aggressive timelines
Strong organization and communication skills
Preferred, not required Bachelor’s Level in Social Work, Psychology, Counseling or related field
