Summary: Provide technical and program support regarding ServicePoint Resources to POINT and PCOC partners. Provide person-centered counseling services to aging adults 55 and older, adult with disabilities and their caregivers. Provide conflict-free information and one-on-one counseling to help them make informed decisions about options available to them.

Duties and Responsibilities:

Manage and respond to all consumer inquiries submitted through the PCOC resource website or contact phone number.
Review and triage all self-assessments completed through the PCOC website to the appropriate agency/provider for consumer follow-up and counseling where PCOC is warranted with- in 48 hours of receipt.
Track and monitor person-centered options counseling referral.
Update and Maintain database policies and internal documents including the PCOC Resource Directory to ensure all Medicaid policies and documents are created and updated as required.
Manage the PCOC phone line, web referrals and self- referrals.
Build and maintain relationships with key vendors including database software developer and Medicaid/PCOC specific vendors.
Provide specialized and seasonal resource updates to team members and community partners.
Train and support team members.
Oversee data base development, maintenance, interfacing and updating, including data specific to Medicaid.
Maintain and update the PCOC and POINT Resource Directory.
Track and monitor PCOC referrals, consumer outreach/ follow-ups and PCOC completions/outcomes through the WellSky CRM system.
PCOC will be reported monthly to OHA and will include:Number of PCOC calls per day by type of inquiry and disposition.
Length of PCOC calls by type of inquiry and disposition.
Number of self-assessments received per day.
Length of time between receipt of self-assessment and PCOC/other referrals within 48 hours from receipt.
Total edits/additions to resource directory related to Medicaid, including community partners, providers and state agency personnel.
Follow scope of work provided by OHA and use it as guide for accomplishments
Attend all required trainings by EOHHS and OHA and assist individuals with materials, as needed
Handle Incoming/Outgoing Contacts/Follow-up
Attend meetings as necessary, regarding the project
Attend outreach events representing The Point and PCOC
Work with team members at the POINT, PCOC the POINT Regional Partners and OHA to refine strategies to address required populations across Rhode Island
Continue with a plan to coordinate and facilitate communication and coordination between SHIP/POINT agencies, MME and BEC
Track all data in ServicePoint and/or Stars
Keep a paper file with supportive documentation for all options counseling cases
Work closely with other Options Counselors and PCOC, under this project, to coordinate activities and responsibilities to ensure statewide access to person-centered counseling for all affected beneficiaries.
Gather input from stakeholders about the effectiveness of the public awareness campaign regarding person-centered counseling regarding Medicare/Medicaid benefits to eligible beneficiaries and make improvements to the campaign, when appropriate
Continue attending  meetings and trainings as required
Coordinate and facilitate communication and coordination between OHA and PCOC team
Keep United Way staff informed of daily activities
Provide monthly written summary of activities
Complete time sheet weekly
Knowledge, Skills, and Abilities:

Certifications Required:

SHIP Certification within six months of hire
AIRS Certification within one year of hire
PCOC Certification within six months of hire
Education, Experience and Skills:

(Technological and soft skills required, education or years of service required)

Associates level in Social Work, Psychology, Counseling or related field and/or experience in working with related field.
Experience and knowledge of Medicare & Medicaid and/or other insurance plans
Experience in working with groups and speaking publicly
Experience in one-on-one counseling and/or client-focused activities
Must have own transportation and be willing to travel throughout the state, as needed
Ability to work independently and to meet aggressive timelines
Strong organization and communication skills
