Family Visitor – Healthy Families America

East Bay Community Action Program (EBCAP) is seeking a full time Family Visitor for our Healthy Families America (HFA) Program located at 100 Bullocks Point Avenue in Riverside, RI. HFA is designed for parents facing challenges, and families are enrolled prenatally or within three months of birth. EBCAP believes all families share the same aspirations for their children and will work collaboratively with the HFA team to contribute to their children’s learning and developing experience.

The successful candidate will provide assessment, intake, home visiting, goal setting, and planning according to the benchmark goals established by the Rhode Island Department of Health. The successful candidate will coordinate services with other EBCAP home visiting, health, and teen-focused groups to maximum services and eliminate duplication. The ideal candidate will adhere to HFA Performance Standards, agency goals, as well as personnel policies.

The position requires a minimum of a Bachelor’s degree in a Human Services field with a minimum of three years’ experience in the field. The ideal candidate must have knowledge of community resources and enjoy working with children and families.
