East Bay Community Action Program (EBCAP) is seeking a full time Assistant Program Manager/Hospital Liaison to work in our Barrington location at 2 Old County Road. The Assistant Program Manager/Hospital Liaison will coordinate communication with key medical and social services (inside and outside of the organization) involved with Integrated Health Home (IHH) or Assertive Community Treatment (ACT) client’s care upon discharge from a higher level of service (such as hospitalization), to potentially reduce medical errors, missed appointments, and dissatisfaction with care.  Encourage client towards direct communication with providers, as well as, preparing clients to eventually self-manage their acute and chronic conditions. Provide supervisory and clinical coordination to a multidisciplinary clinical and rehabilitative staff engaged in the delivery of intensive community-based services to an adult population living with severe and persistent mental illness.

EBCAP is offering a two-step sign-on bonus totaling $2,000 for external applicants.

CORE POSITION FUNCTIONS IN SUPPORT OF ASSISTANT PROGRAM MANAGER:

  • Under the direction of a Program Manager, supervise a multidisciplinary team working to integrate services for clients with severe and persistent mental illness, many of whom are dually diagnosed with substance abuse issues.
  • May conduct daily team meetings to plan and review services for enrolled clients and to ensure the provision of quality and integrated care.
  • Provide ongoing education to staff to ensure that quality care includes the interaction of medical, pharmacological, psychosocial, spiritual and vocational issues.
  • Maintain positive working relationships with other Managers, eliciting the involvement of appropriate personnel in the supervision of team RN’s and Vocational Specialists.
  • Continuously monitor the content and timeliness of team members’ clinical documentation, addressing concerns as they arise, and ensuring compliance to the Center’s published standards for the completion of clinical documentation.
  • Ensure team members’ adherence to productivity expectations.
  • Participate in the recruitment, screening, and hiring of team members. Provide orientation to new hires under direction of Program Manager.
  • In the absence of Program Manager, assume responsibility for all administrative and clinical duties of the respective team.
  • Keep Program Manager informed of significant information during regular supervision or on an ad-hoc basis, as needed.
  • Assist Program Manager in the adherence to, and achievement of, annual budget.

Must have a bachelor’s degree in human services field.  Minimum of two (2) years’ experience in community-based treatment and rehabilitation.
