East Bay Community Action Program (EBCAP) is seeking a full time Nurse Care Manager for our Health Center at 6 John Chafee Boulevard in Newport, RI. We are seeking highly professional and motivated individual who enjoys working with a multi-disciplinary team in a community health setting.

Due to the critical nature of this position to both the community and the organization, EBCAP is offering a two-step sign on bonus totaling $1,000. 

The successful candidate will provide comprehensive screenings, assessments, care coordination services, disease education and self-management support to specific patient populations. Nursing care is performed in both office-based and community-based settings, depending on the needs of the patient. Duties include, but are not limited to: provide ongoing health assessment (identify health issues, behaviors, needs, barriers, etc.) and all other assessments which are appropriate for the nursing scope of practice; collaborate and regularly liaise with internal/external providers, internal/external pharmacies, internal/external labs, and community agencies, such hospitals, long-term care facilities, and home health representatives based on consumer’s health and wellness needs; record and administer medications prescribed by the team physician/psychiatrist; evaluate and chart medication effectiveness, complications, and side effects; complete Prior Authorization procedures based on patient’s insurance requirements as needed. He/she will also identify “high risk” patients, and utilizes a multi-disciplinary Primary Centered Medical Home (PCHM) care team approach to address opportunities to plan and coordinate care, as well as arrange additional internal/external support services, such as contact with Health Advocates, to treat the patient holistically.

The successful candidate must have a minimum of an Associates’ Degree in Nursing or equivalent, and licensed as a Registered Nurse (RN) by the RI Department of Health. Two or more years of experience in a community health setting, acute care, or emergency/urgent care working with primary care providers to coordinate care and disease management required. Previous experience in an electronic medical record (EMR) preferred; however, training is provided in EBCAP’s EMR which is NextGen.
