Nurse Care Coordinator $1,000 Sign-On Bonus (EBCAP1037)
East Bay Community Action Program (EBCAP) is seeking a full-time Nurse Care Coordinator for our Health Center at 6 John Chafee Boulevard in Newport, RI. We are seeking a highly professional and motivated individual who enjoys working with a multi-disciplinary team in a community health setting. The Nurse Care Coordinator will function as a key staff member to coordinate collaborative initiatives between EBCAP and NCCMHC. Functions are co-located at NCCMHC and EBCAP/Chafee, as well as providing services in the community. This position will also coordinate treatment planning and primary care/behavioral health care consultation for shared clients, appropriately record clinical and administrative documentation in approved Electronic Health Record (EHR) and participate in quality improvement activities and outcomes reporting in compliance with Van Buren grant requirements.
EBCAP is offering a two-step sign-on bonus totaling $1,000 to external applicants.
The successful candidate must have a minimum of an associate degree in nursing or equivalent, and licensed as a Registered Nurse (RN) by the RI Department of Health. Two or more years of experience in a community health setting, acute care, or emergency/urgent care working with primary care providers to coordinate care and disease management required. Previous experience in an electronic medical record (EMR) preferred; however, training is provided in EBCAP’s EMR which is NextGen.
