East Bay Community Action Program (EBCAP) is seeking 3 full-time Screening Support Facilitators, currently located in Barrington, Riverside, and Newport, RI.

EBCAP is offering a two-step sign-on bonus totaling $1,000 to external candidates.

As an awardee of SAMHSA grant funding, East Bay Community Action Program’s (EBCAP) behavioral health services is required to collect and report performance data using the designated Government Performance and Results Act (GPRA) Client Outcomes Measures. This tool is used to survey clients seeking opioid use disorder treatment and collect data to be entered into the SPARS grant database.

The Screening Support Facilitator will work closely with EBCAP’s health center clients by performing various screenings to determine clients’ needs with respect to healthcare and/or social determinants.  The screening support Specialist(s) will enter screenings into our electronic health record as well as enter the GPRA screenings into the SPARS database.  The individual will also refer to appropriate services internally and externally as needed.

High School diploma or equivalent (GED) required.  Medical vocational/technical training preferred to work with screening tools and understand clinical terminology.  Must be able to navigate an electronic medical record.
