Case Manager RI Works $1,000 Sign-On Bonus

East Bay Community Action Program (EBCAP) is seeking a full-time Case Manager for our RI Works Program at 100 Bullocks Pt, Riverside, with some travel to Newport, RI. The CM will provide case management and care coordination services to families receiving RI Works (a program managed by the RI Department of Human Services, DHS), ensuring their compliance with specific component areas and assisting with resolving barriers to progress.  The program is designed to encourage families to work and become self-sufficient.

EBCAP is offering a two-step sign-on bonus totaling $1,000 for external applicants.

CM will conduct comprehensive assessments (including assessing risks such as, but not limited to, basic human needs, vocational/employment factors, mental health/substance use, and other related risks) with assigned clients. Develop a service plan with clients in accordance with provisions stated within RI Works Program. Provide office, community, and/or home visitation with assigned clients. Provide mentoring in adult daily living skills as needed and as related to barriers to academic/vocational/employment goals. Assist with the completion of emergency funding, housing, utility assistance, medical and state benefits, and other appropriate applications for clients as needed. Maintain regular client contact as deemed by the program/DHS. Maintain timely and accurate record-keeping for client files in accordance with the program, licensing, accreditation, and organizational standards. Utilize computer database systems to maintain appropriate information on clients, per program and organizational requirements.

The educational requirement for a Case Manager requires a Bachelor’s Degree in Social Work or a related field.
