Summary: Provide person-centered counseling and follow-up services to individuals in quarantine and isolation from COVID_19.  Provide conflict-free information and one-on-one follow-up counseling to help QI make informed decisions about options available to them.

 

Duties and Responsibilities:

 

Follow scope of work provided by RIDOH and use it as guide for accomplishments
Attend all required trainings by RIDOH and assist individuals with materials, as needed
Handle Incoming/Outgoing Contacts/Follow-up within 24 hours
Attend bi-weekly team meetings
Work collaboratively with team members within Senior & Disability Services and 2-1-1
Coordinate and facilitate communication between the Rhode Island Department of Health and QI clients
Track all data in ServicePoint
Keep QIRC team informed of daily activities
Provide written summary of activities as requested
Follow the Quarantine & Isolation procedure and protocol as provided by RIDOH
Follow the Quarantine & Isolation script as provided by RIDOH
Provide information on vaccine locations, community referrals and vaccine informational resources
Provide assistance with scheduling vaccine appointments
Track and record appointments made within internal data systems
Provide standard set of options:
Connection to food and other essential items including delivery of these items,
Housing security and options,
Support for caregivers and childcare,
Mental health referrals and support,
Benefits enrollment assistance,
Wellness checks including social engagement,
Information and help with healthcare,
Additional needs can also be addressed.
Knowledge, Skills, and Abilities:

 

Professional training and/or experience in Social Work, Psychology, Counseling or related field
Experience in working with groups
Experience in one-on-one counseling and/or client-focused activities
Must have own transportation
Ability to work independently and to meet aggressive timelines
Strong organization and communication skills
Preferred, not required Bachelor’s Level in Social Work, Psychology, Counseling or related field
